After nearly two years of proudly serving the Upper Valley, HealthFirst Family Care Center has concluded operations at our Canaan location, located within the Mascoma Community Health Center, on October 31, 2025.

This decision was not made lightly. Due to ongoing changes in Medicaid reimbursement and federal funding for community health centers, high overhead costs, and persistently low patient volume, continuing operations in Canaan was no longer financially sustainable.

We are deeply grateful for the support and trust of the Canaan community during our time here. While our physical location has closed, our medical and behavioral health providers have remained with HealthFirst, having transitioned to our Franklin and Laconia offices, continuing to care for our patients.

For those who prefer not to travel to Franklin or Laconia, we have partnered with Mid-State Health Center, another Federally Qualified Health Center (FQHC), which operates locations in Bristol and Plymouth. Our team is happy to assist with referrals or transferring care as needed.

Additionally, HealthFirst’s Behavioral Health team has continued providing school-based services in the region to support the mental health needs of local students.
