HealthFirst Family Care Center emphasizes providing access to primary care services to everyone, regardless of their ability to pay and works with our community partners to achieve our mission.
Providing Integrated Care
We provide care, compassion, and support to the whole person. By providing integrated health care we join our mental health, substance use disorder, primary health care, and care coordinators to create an effective approach towards caring for people in need.
Offering Payment Solutions
HealthFirst will never deny access to services because of one’s inability to pay. Should you be experiencing financial hardship, we will gladly discuss our discounted sliding fee scale, which is available based on family size and income.
Our Patient Advocates will assist you with options to reduce your out-of-pocket costs. The Patient Advocate will review your current income status and help guide you to options like, Health Insurance through the Market Place, NH Easy programs, Medicaid, and our sliding fee discount program.
Team Centered Approach
Your HealthFirst team is led by your Primary Care Provider (PCP) and may include nurses, medical assistants, counselors, and/or nutritionists, as well as specialists from other agencies.
