Clinical Quality Assurance Manager (CQAM)
JOB SUMMARY: HealthFirst, a small Federally Qualified Health Center in the Lakes Region, is hiring a Clinical Quality Assurance Manager (CQAM). This position will report to the President/CEO and the Medical Director for the overall QA/QI program. The CQAM will assure that the team members are planning and implementing chronic care activities in a coordinated fashion using PDSA cycles, share best practices from national and local programs as part of the project team to develop and implement Electronic Patient Registries. The CQAM will be responsible for improving client self-management goals, facilitate project planning and implementation, provide consultation and technical assistance to staff members, gather and analyze quality assurance data with the medical and administrative staff and help the agency with achieving NCQA, Medical Home and Meaningful Use certifications. The position requires a BSN or NH nursing license or MSN. The CQAM will have the ability to work with a team to develop quality measures and training personnel. Clinical Nurse Supervisor
HealthFirst, a small Federally Qualified Health Center in the Lakes Region, is looking to hire a Clinical Nurse Supervisor. This position works directly with Clinicians, Nurses and Medical Assistants to ensure the day-to-day operations and to help enhance the quality of patient-centered care. This position will work out of both Franklin and Laconia locations. Flexibility to work a variety of schedules, including late nights up to 7pm. Duties Include but not limited to: Directing, organizing and supervising the daily work of clinical staff which includes RNs, LPN, CMAs and RMAs; go-to person for day-to-day clinical activities; staff training; ensuring completion of annual lab competencies; help problem solve and exercise critical thinking skills to ensure the daily operations of the clinic are completed. Education/License & Certification Requirements: Current, unrestricted NH RN license; 3-5 years of supervisory experience; BLS certification. BSN strongly preferred.

This position will provide social work in the medical office setting performing intakes and assessments, referring to the behavioral health team, assisting clients through advocacy, enabling services, applications for various local, state and federal benefits. To assist clients in their plan of care, this position will also utilize referral mechanisms to connect clients with service agencies in the community.
Position will be shared between Franklin and Laconia offices alternating days between sites.
The desired candidate will have a Master’s Degree in social work or behavioral health counseling. A Bachelor’s degree and four years or more of experience may be considered under some circumstances.
